Starfield Primary Care provides mobile medical services in the San Fernando Valley, delivering care in a variety of assisted living facilities, supportive housing programs, and homeless shelters. In addition to mobile care, our team offers in-office medical visits at our Sunland Medical Office, located at 8055 Foothill Blvd, Sunland, CA 91040.

Meet the San Fernando Valley Team

Steven Hochman, MD, MPH

Steven Hochman, MD, MPH, is a board-certified general internist and addiction medicine specialist. He completed medical school at the Keck School of Medicine of USC and residency training at the University of North Carolina. Dr. Hochman previously served as a staff physician and Medical Director at JWCH Community Health Center in Los Angeles and later as a staff physician and Medical Director at Akido Labs, an innovative medical group focused on technology-enabled care delivery.

Dr. Hochman will join Starfield Primary Care in March 2026, providing care in field-based settings including assisted living communities and supportive housing sites, as well as office-based care at Starfield’s Sunland location. He lives in Los Angeles with his wife and three daughters.

Sapir Tabibi, PA-C

Sapir Tabibi is a Physician Assistant specializing in primary care who is strongly committed to providing high-quality, patient-centered care. She earned her bachelor’s degree from UCLA before obtaining her Master’s in Physician Assistant Studies from Elon University.

Sapir is passionate about building lasting relationships with patients and helping them achieve their health goals through education, advocacy, and accessible care. With a strong interest in preventive medicine, Sapir is particularly interested in assisting patients to build healthy lifestyles to prevent and manage chronic diseases.

Michele Rodgers, Director of Care Management Operations, Los Angeles Region

Michele Rodgers joined Starfield in 2026 as a Lead Care Manager focused on serving the San Fernando Valley community. She brings over a decade of experience in health plan operations, client services, and complex care coordination, including six years at Health Net. She also previously started her own notary business. Michele works closely with Starfield’s clinical team to ensure patients receive coordinated, compassionate, and efficient care.

Jack Benson, Care Manager

Jack Benson is an experienced case manager with more than 20 years of experience working with individuals who have complex mental health and social needs. He provides care navigation and guidance for Starfield’s San Fernando Valley patient population in close collaboration with the medical team. Jack also serves as Starfield’s Lead Care Manager for Enhanced Care Management services for Los Angeles County Medi-Cal programs.

Terence Yu, MSW

Terence is an experienced Social Worker who has been providing services to communities for over 15 years to assist vulnerable populations live their fullest potential.  He is committed toward empowering others and advocating for patient rights by way of providing psychoeducation and implementation of evidence based practices while strongly adhering to Social Work values.  Terence also has a unique understanding of multiple complex social systems in place that simultaneously creates barriers, and he has a particular interest in helping those navigate through such structures.

Terence completed his MSW at Arizona State University with prior experiences in tackling the grand challenges of ending homelessness and closing the health gap.

Jose “Castro” Castro, Care Manager

Jose Castro, who goes by “Castro,” is an experienced case manager dedicated to supporting vulnerable, high-need patient populations with complex medical and social challenges. He works closely with patients to address barriers to care, connect them with community resources, and help them navigate complicated health and social service systems.

Before joining Starfield Primary Care, Castro served as a supervisor at Healthcare in Action, an organization providing mobile medical and care management services for people experiencing homelessness. He brings extensive experience supporting individuals with complex needs and is passionate about building trusting relationships and helping patients achieve greater stability and well-being.

Karina Valadez, Care Manager

Karina Valadez joined Starfield Primary Care in 2026 as an Enhanced Care Manager serving the San Fernando Valley community. She brings extensive healthcare experience in care coordination, member advocacy, authorizations, eligibility, and provider relations, along with a strong understanding of Medi-Cal, Medicare, and HMO systems.

Karina works closely with Starfield’s clinical team to help patients navigate complex healthcare needs, overcome barriers to care, and connect with medical, behavioral health, and community resources. She is passionate about supporting patients with patience, compassion, and respect, and takes pride in helping each person feel heard, supported, and connected to the care they need.
